A Sports, or Camp Physical—also known as a pre-participation physical exam—is required by many schools, athletic programs, and summer camps to ensure a student or athlete is healthy and medically cleared to participate. These physicals help identify any conditions that may affect safe participation in organized sports, or camp programs.
At Physicals Plus Healthcare, we provide walk-in sports, and camp physicals with no appointment required.
Our medical providers review medical history, perform a comprehensive head-to-toe physical exam, and make recommendations when clinically appropriate.
Our clinic focuses on healthy visits only, so patients are not exposed to sick individuals. If your primary care provider is overbooked, you can simply walk in and complete your physical quickly and efficiently.
** Sports and camp physicals at Physicals Plus Healthcare are intended to assess medical readiness for participation and are not a substitute for an annual comprehensive physical exam with your primary care provider.
The Nurse Practitioner will obtain a comprehensive medical history, including medications, allergies, current and past conditions, and family medical history. This information should be completed by both the participant and a parent or guardian.
The physical exam includes a comprehensive evaluation of the athlete’s overall health and readiness for participation.
This assessment consists of:
Measuring height, weight, and vital signs, including heart rate, blood pressure, respiratory rate, and oxygen saturation.
A head-to-toe examination is performed:
Including vision screening of the eyes, assessment of the ears, nose, and throat, and evaluation of the lungs and heart to ensure proper respiratory and cardiovascular function. The abdomen and skin are examined, followed by a musculoskeletal assessment to evaluate strength, flexibility, posture, and joint stability.
A focused neurologic assessment is performed when there is a history of concussion or head injury.
Upon completion of the exam, a signed medical clearance form will be provided for you to take home for sports, or camp participation.

Some athletes may need a clearance letter from a medical specialist before being fully cleared for sports or camp participation. This is typically required when there is a medical history that could affect safety during physical activity. Clearance decisions are made on an individual basis, depending on the condition, severity, and current level of control.
